Our Story

As a preferred provider of outpatient physical therapy with multiple locations in Orange County, Inland Empire, and Los Angeles, we saw a significant need for quality home health services. We encountered numerous patients visiting our offices who were not properly prepared for outpatient therapy. To address this need, a new home health service company was formed — C.O.R.E At Home. 

 

Our compassionate and deeply experienced team of providers assists patients in returning to their normal level of function in the comfort of their own homes. We are intimately aware of the importance of the continuum of care from injury or surgery to home care, outpatient services, and back to the normalcy of life. Because of our multi-faceted healthcare experience, we can make this transition simpler, more comfortable, and more efficient for each patient.
